A thorough understanding of the principles, clinical application, risks, and benefits associated with VisionCare’s Implantable Miniature Telescope (by Dr. Isaac Lipshitz) is necessary before using this product. Please read the entire Physician Use Information booklet. Provide the Patient Information Booklet to candidate patients and advise them to read or have it read to them by a friend or family member. Give the patient ample time to consider whether to have the procedure. Please encourage and provide candidate patients the opportunity to ask you questions and to ask questions to their referring ophthalmologist or other eye care professionals. The patient and the implanting physician must complete an Acceptance of Risk and Informed Decision Agreement before telescope implantation surgery
